WHAT MOST INSURANCE COMPANIES REQUIRE:
FOR INITIAL EVALUATION AND DIAGNOSTIC STUDIES:
No pre-authorization is required and no physician referral is required. All Insurance companies provide benefit for initial evaluation and ultrasound venous testing.
VARICOSE VEIN PROCEDURES:
Many insurance companies have increasingly strict criteria and may require positive answers to the following:
- A trial of conservative management. This usually means wearing support stockings for three months.
- Symptoms severe enough to affect your ability to work, engage in daily activities. For instance, you might relate that your symptoms keep you from exercising, or make you take more frequent breaks than the rest of your fellow employees, or you are limited by what you can do after work because of leg fatigue or soreness. You may relate a sleep disturbance from residual leg pain.
- Taking pain medications for symptoms. Pain medication can include Tylenol, ibuprofen (eg. Advil and Motrin), or naproxen (eg, Aleve)
HOW WE WORK WITH YOUR INSURANCE COMPANIES: We will obtain authorization for required procedures. Patients with insurance are never asked for payment up front. We will bill your insurance company and after they submit their payment, we will bill for your co-pay and/or deductible.
